Lock’s Hill Surgery want your views

This consultation deadline has been extended to 23rd September 2016. There are hardcopies you can pick up from our offices or download one from our website here: Practice_consultation_document_(2).pdf

This proposal for closure cannot be sanctioned without NHS England approval.

Frome Medical Practice wants to fully consult with patients to seek views on the proposal.

Set out below are just some of the issues that they have considered in this proposal:

  • Appointment availability
  • Safety and quality of service
  • Efficiency
  • The Future for Patients Normally Attending at Locks Hill

What Happens Now?

If you would like your say on this proposal, then please complete and return the attached form and hand it in either to the reception at the Locks Hill surgery or alternatively to the Information Desk at the main surgery at the Frome Medical Centre, Ecos Way.
