Lock’s Hill Surgery want your views

Frome Medical Practice is considering a proposal to close its branch surgery on Locks Hill. 

This consultation deadline has been extended to 23rd September 2016. There are hardcopies you can pick up from our offices or download one from our website here: Practice_consultation_document_(2).pdf  

This proposal for closure cannot be sanctioned without NHS England approval.

Frome Medical Practice wants to fully consult with patients to seek views on the proposal.

Set out below are just some of the issues that they have considered in this proposal:

  • Appointment availability
  • Safety and quality of service
  • Efficiency
  • The Future for Patients Normally Attending at Locks Hill

What Happens Now?

If you would like your say on this proposal, then please complete and return the attached form and hand it in either to the reception at the Locks Hill surgery or alternatively to the Information Desk at the main surgery at the Frome Medical Centre, Ecos Way. All forms must be completed and returned to us by 19th August 2016. The medical practice will keep you fully informed of any progress via their practice website, Twitter account, reception screens and posters.
